How It Works:

Step 1: Kick things off with our Patient Weight Loss Intake Form. It’s a simple start to understanding your health needs and goals.

Step 2: If you’re provisionally eligible, complete the ProMedX Weight Loss Membership Contract and get your patient profile built on our secure, HIPAA-compliant EMR.

Step 3: Get your lab work done. We’ll order it, you’ll provide the blood sample, and we’ll analyze the results.

Step 4: Our providers will review your results and prescribe the Semaglutide/B12 medicine if medically appropriate. We’ll also refer you to Planet Nutrition for a body composition analysis and provide you with a comprehensive weight loss packet.

Step 5: Your prescription will be shipped to you, making it ultra-convenient. We’ll even provide you with an easy-to-follow Semaglutide Injection Instruction Sheet.

Step 6: For your monthly check-ins, visit our clinic. Track your progress with the provided sheets and head to Planet Nutrition every 3 months for a body composition analysis. Our goal is to help you until you achieve your desired results.
